1. If you have Aetna, UHC, Humana, Multiplan, PNOA, VA OptumEast, Premera, Regence, or UMR check insurance coverage: Here

- If you do not have a policy I can bill to, you may self pay for an appointment with a debit/credit card or FSA/HSA card and I will provide a coded, superbill you can then submit directly to your insurance carrier for reimbursement.
